Er mwyn gallu talu breindaliadau, mae’n hanfodol fod aelodau Eos yn llenwi’r ffurflen fanylion. Ni fydd Eos yn gallu gwneud taliad i aelod heb y ffurflen hon. Gellir ei lawr lwytho fan hyn:

CADARNHAU COFNOD EICH GWEITHIAU
Gan mai PRS sy’n casglu’r wybodaeth ar ddefnydd gweithiau Eos, mae’n hanfodol fod eu bas data yn cynnwys cofnodion cywir o holl weithiau aelodau Eos. Byddai unrhyw wall neu gamgymeriad yn bas data PRS yn golygu na fydd y gwaith yn cael ei gynnwys ym mhroses ddosrannu Eos.
Er mwyn diogelu fod pob aelod yn cael eu talu yn gywir, gofynnwn i chi edrych ar gofnodion eich gweithiau sydd ar fas data PRS a chadarnhau fod y wybodaeth yno’n gywir. Os oes unrhyw wybodaeth yn anghywir neu ar goll, mae modd i chi addasu cofnod eich gweithiau ar y bas data.
Os nad ydych chi wedi cofrestru i gael mynediad i fas data PRS, mae manylion ar gael fan hyn.
Os ydych chi eisiau addasu’r wybodaeth sydd ar y bas data, mae manylion ar gael fan hyn ar dudalen gymorth Gwasanaethau Ar-lein PRS. Bydd rhaid cofrestru i gael mynediad i fas data PRS cyn medru addasu unrhyw wybodaeth berthnasol yn ogystal â chofrestru unrhyw weithiau newydd.
Os oes gennych chi gatalog mawr o weithiau ac mae’n anymarferol i chi gadarnhau pob cofnod (e.e. os ydych chi’n gyhoeddwr) cysylltwch yn uniongyrchol â ni yn y swyddfa.
MEMBER INFORMATION
In order to be able to pay royalties, it is essential that Eos members complete the details form. Eos will not be able to make a payment to a member without this form. It can be downloaded here:

CONFIRM THE RECORD OF YOUR WORK
As PRS collects the information on the use of Eos' works, it is essential that their database contains accurate records of all the work of Eos members. Any error or error in the PRS database would mean that the work will not be included in the Eos apportionment process.
In order to ensure that all members are paid correctly, we ask that you check the records of your works on the PRS database and confirm that the information is correct. If any information is incorrect or missing, you can modify your work record on the database.
If you have not registered to access the PRS database, details are available here.
If you want to modify the information on the database, details are available here on the PRS Online Services help page. You will have to register to access the PRS database before any relevant information can be adapted as well as registering any new works.
If you have a large catalog of works and it is impractical for you to confirm all entries (eg if you are a publisher) please contact us directly at the office.
